Only use this form if the court issued your application before 6 April 2022. If your application was issued on or after 6 April 2022, you will need to use a different form.

If you’re applying for an order to protect you or a child from harm, you must not hand the court papers to the respondent yourself. You can ask a court bailiff to do it for you using this form. There’s no fee involved.

You can also use this form if you need the court’s help to deliver papers about your application for a divorce, dissolution of a civil partnership or (judicial) separation.
